Medical Release: In case I/we cannot be reached during an emergency, I/we the undersigned give permission for my/our child to be treated by a licensed physician if this emergency might endanger his/her life and/or cause disfigurement, physical impairment or undue discomfort by delaying treatment. Said physician is to administer whatever care is necessary, including anesthesia. The undersigned assumes responsibility for any costs connected with such treatment and hereby releases Awana Clubs International, First Baptist Church of Lakewood, Washington and the driver of any vehicle transporting my child to a supervised Awana outing, from liability. This release form is completed and signed of my/our own free will and with the sole purpose of authorizing medical treatment under emergency circumstances in my/our absence.

Photo Release: During Awana Club, photo/video may be taken of our volunteers and children as they learn & play. Some of these photos may be used for church/children's ministry promotional purposes. Please note that declining to give permission to publish images of your child online does not mean that they will not be photographed during Awana. Due to the nature and size of our Awana Club, it is impossible to guarantee their photo will not be taken. If you withhold permission for online publication, we won’t post a photograph or video in which your child is included. We will never identify children included in a photograph or video without express permission. I understand that as an Awana participant, my child may be photographed or videotaped in connection with Awana and these photos/videos may be used in promotional materials or on FBC Lakewood's website.  I do hereby give permission for my child’s photos/videos to be used by FBC Lakewood's Preschool / Children’s Ministries.
